RESUMO Objetivo: Avaliar a adesão a três métodos de introdução alimentar aos 7 meses de vida. Métodos: Ensaio clínico randomizado com pares mães-lactentes submetidos à intervenção aos 5 meses e meio de vida sobre três diferentes métodos de introdução alimentar, conforme randomização: tradicional, Baby-Led Introduction to SolidS (introdução de sólidos guiada pelo bebê, em português) ou misto (criado especialmente para este estudo). A adesão ao método foi avaliada aos 7 meses em ligação telefônica feita para o cuidador por pesquisador cego em relação ao método. As análises foram realizadas por teste do qui-quadrado e os dados apresentados em número absoluto e percentual. Resultados: Foram avaliados 139 pares mães-lactentes, 46 alocados no método tradicional, 47 no Baby-Led Introduction to SolidS e 46 no misto. Aos 7 meses, 60 (43,2%) mães relataram que seus lactentes seguiam o método alimentar proposto. Analisando-se cada abordagem, o método misto apresentou maior probabilidade de adesão (71,7%, n=33), seguido do tradicional (39,1%, n=18) e de introdução de sólidos guiada pelo bebê (19,2%, n=9) (p<0,001). Da amostra que não seguiu o método proposto, aqueles que haviam sido randomizados para os métodos tradicional e de introdução de sólidos guiada pelo bebê migraram majoritariamente para o misto (92,9%; n=26 e 92,1%; n=35, respectivamente) (p<0,001). Conclusões: A alimentação complementar em abordagem mista obteve maior adesão aos 7 meses de idade.

Abstract Objective: To assess the adherence to three methods of food introduction for 7-month-old babies. Methods: This is a randomized clinical trial conducted with mother-infant pairs, submitted to the intervention with five and a half months of age and three different methods for food introduction according to randomization: Parent-Led Weaning (PLW), Baby-Led Introduction to SolidS (BLISS), or mixed (specially developed for this study). Adherence to the method was assessed at the seventh month of age, via telephone call to the caregiver by a researcher blinded to the method. The analyses were performed using the Chi-Square test and data are presented in absolute numbers and percentages. Results: A total of 139 mother-infant pairs were evaluated; 46 of them were allocated to the PLW method; 47, to the BLISS; and 46, to the mixed. At seven months of age, 60 (43.2%) mothers reported that the infants were following the proposed feeding method. When analyzing each approach, the mixed method showed a higher likelihood of adherence (71.7%, n=33), followed by the PLW method (39.1%, n=18) and by the BLISS (19.2%, n=9) (p<0.001). Among the sample that did not follow the proposed method, those that had been randomized to the PLW and BLISS methods mostly migrated to the mixed method (92.9%; n=26 and 92.1%; n=35, respectively) (p<0.001). Conclusions: Complementary feeding in a mixed approach obtained greater adherence in 7-month-old babies.

RESUMO Objetivo Verificar as alterações no estado nutricional em adolescentes de 10-19 anos, observadas após 12 meses, no mínimo, da conclusão de tratamento oncológico de leucemias e linfomas. Métodos Trata-se de estudo quantitativo de delineamento longitudinal realizado no Hospital de Clínicas de Porto Alegre. Foram incluídos adolescentes de 10-19 anos, que haviam concluído tratamento oncológico para leucemias e linfomas há no mínimo doze meses. Foram coletadas as medidas de peso, estatura, circunferência braquial, dobra cutânea tricipital, circunferência muscular do braço e circunferência abdominal. Resultados A amostra foi constituída por cinquenta adolescentes sobreviventes de leucemias e linfomas. No follow up, 38% dos pacientes foram classificados como excesso de peso de acordo com o índice de massa corporal para a idade. Houve um aumento significativo no índice de massa corporal para a idade, ocorrida entre o início do tratamento, seu fim e follow up (p=0,013) nos indivíduos do sexo feminino, quando comparados a indivíduos do sexo masculino. Os resultados apontam uma diminuição nas médias de Escore-Z de estatura para a idade, com diferenças significativas entre o início do tratamento e follow up (p=0,016) e entre término do tratamento e follow up (p=0,006), em pacientes de ambos os sexos. Conclusão Os indicadores antropométricos demonstram uma frequência importante de excesso de peso e aumento da dobra cutânea tricipital, um aumento significativo do índice de massa corporal para a idade, bem como um deficit de crescimento entre os sobreviventes.

ABSTRACT Objective To examine the changes in the nutritional status of adolescents aged 10-19 years after a minimum 12 months interval following oncological treatment for leukemias and lymphomas. Methods Longitudinal design quantitative study conducted at Hospital de Clínicas, Porto Alegre. Adolescents aged 10-19 years after a minimum 12 months interval following oncological treatment for leukemias and lymphomas were included. The measures of weight, height, brachial circumference, triceps skinfold thickness, arm muscle circumference and abdominal circumference were collected. Results The sample comprised 50 adolescents who had survived leukemias and lymphomas. In the follow up 38% of the patients were classified as overweight according to the body mass index for their age. There was a significant increase in body mass index for age between the beginning and the end of treatment and follow up (p=0.013) in female individuals, compared to males. The results indicate a reduction in the Z-score means of height for age, with significant differences between the beginning of treatment and follow up (p=0.016); and end of treatment and follow up (p=0.006) in patients of both genders Conclusion The anthropometric indicators show an important frequency of excess weight and increased tricipital skinfold, as well as a significant increase of the body mass index for age and also a growth deficit among the survivors.

RESUMO Objetivo Avaliar o estado nutricional e capacidade funcional de pacientes adultos oncológicos hospitalizados. Métodos Estudo transversal em pacientes oncológicos adultos do Hospital de Clínicas de Porto Alegre. Os pacientes foram divididos de acordo com a presença de Tumores Sólidos e Tumores Hematológicos. O estado nutricional foi identificado pela Avaliação Subjetiva Global Produzida pelo Paciente e a capacidade funcional pela Força do Aperto de Mão – medida por dinamometria – e pelo Índice de Desempenho do Eastern Cooperative Oncology Group. Resultados Foram avaliados 76 pacientes (56±17 anos, 35,5% do sexo feminino), 63,2% apresentaram Tumores Sólidos e 36,8% Tumores Hematológicos. De acordo com a Avaliação Subjetiva Global Produzida pelo Paciente, 53,9% dos pacientes estavam moderadamente e gravemente desnutridos e demonstraram baixa capacidade funcional de acordo com a Força do Aperto de Mão e Índice de Desempenho do Eastern Cooperative Oncology Group, 47,9% e 32,2%, respectivamente. Os instrumentos de capacidade funcional demonstraram uma concordância moderada (Kappa=0,427; p<0,001) e correlação positiva (r=0,136; p=0,028). Pacientes gravemente desnutridos demonstraram ter uma menor Força do Aperto de Mão quando comparados aos bem nutridos (24,0±10,4 vs. 34,2±16,6kg; p=0,015). Resultados foram confirmados entre pacientes moderadamente e gravemente desnutridos que apresentaram Força do Aperto de Mão abaixo do percentil 40, considerado uma baixa capacidade funcional. Conclusão Neste estudo, os pacientes oncológicos hospitalizados, independentes do tipo de tumor, apresentaram comprometimento do estado nutricional e baixa capacidade funcional. A Avaliação Subjetiva Global Produzida pelo Paciente identifica de forma mais precoce a necessidade de uma intervenção nutricional especifica. Ainda, a Força do Aperto de Mão deve ser considerada para complementar a avaliação nutricional neste grupo de pacientes.

ABSTRACT Objective To evaluate the nutritional status and functional capacity of hospitalized adult patients. Methods Cross-sectional study of adult oncology patients at Hospital de Clínicas de Porto Alegre. Patients were evaluated according to Solid Tumors and Hematologic Tumors. The nutritional status was obtained using Patient Generated Subjective Global Assessment, and the functional capacity was evaluated by Handgrip Strength using a Jamar® dynamometer – and the Performance Index of the Eastern Cooperative Oncology Group. Results This study evaluated 76 patients (56±17 years old, 35.5% female), 63.2% with Solid Tumors and 36.8% with Hematologic Tumors. According to the Patient Generated Subjective Global Assessment, 53.9% of the patients were moderately and severely malnourished and demonstrated functional capacity, according to the Handgrip Strength and Performance Index of the Eastern Cooperative Oncology Group, of 47.9% and 32.2%, respectively. The functional capacity instruments showed a moderate agreement (Kappa=0.427, p<0.001) and positive correlation (r=0.136, p=0.028). Severely malnourished patients had a lower Handgrip Strength when compared to well nourished (24.0±10.4 vs. 34.2±16.6kg, p=0.015). The results were confirmed among moderately and severely malnourished patients, who were rated at the 40 percentile, considered low functional capacity. Conclusion In this study, hospitalized oncological patients presented poor nutritional status and low functional capacity. The Patient Generated Subjective Global Assessment identifies the nutritional specification earlier. In addition, Handgrip Strength dynamometry can be a useful tool to evaluate the low functional capacity and nutritional status. It can be included in cancer patient’s evaluation, along with other nutritional assessment tools.
